This Privacy policy outlines why and how personal and sensitive information is collected, handled and shared by Principle Psychology Practice, in order to be compliant with the requirements of the General Data Protection Regulation (GDPR). Data will only be kept for as long as required. Information will solely be kept for the purpose it was intended.
The type of data collected includes background information from parent and school staff (with parental consent) that can be used to inform regarding early development and previous educational history. Information will be collected about a young person's strengths, interests and the current concerns of the referrer.
When a young person is assessed, information will be gathered from observation and consultation with setting staff, the young person, and perhaps by obtaining samples of work. Information may also be gathered during more standardised assessments. The information collated will then be incorporated into the confidential educational psychology report.. A signed copy of the report will be sent to the referrer.
All the information gathered will be treated and stored confidentially. It will only be accessed solely for the purpose of work as an Educational Psychologist. Information will not be shared informally via any means (electronically or otherwise) unless consent has been obtained by the referrer.
Aspects of anonymised data may be shared with other independent psychologists as part of regular professional supervision, in line with HCPC registration requirements.
However, it should be noted that in specific circumstances, for example, relating to safeguarding concerns The Data Protection Act (1998) does allow personal data to be disclosed to other relevant professionals, therefore under such circumstances I would be legally obliged to disclose data.
Information will be stored securely, paper versions of the report will be sent to the referrer and a copy retained by the referree for 10 years, in case further copies are required in the future. As a parent or legal guardian of a young person under the age of 16 years, or a young person over the age of 16 years, you do have the right to request a further copy of the report produced or request information held is deleted prior to the specified time frame mentioned above, by sending a written or typed, signed request to Dr Sharon Lourd-Moyo.
Invoice information and appointment diaries will be kept for 7 years in line with Tax Regulations.
